Director of Case Management - Acute Care Hospital — Greater Boston Market
The Opportunity
- Ready to lead a case management team that genuinely moves the needle on patient care and hospital performance? A well-established acute care hospital in Massachusetts is looking for a dynamic Director of Case Management to take the reins of a department that touches nearly every patient who comes through its doors.
- This is a sizable, full-service hospital — licensed for nearly 300 beds, with roughly 270 in active daily use, a large emergency department equipped with specialized treatment rooms, and a dedicated short-term behavioral health unit serving adolescent and adult patients. It's the kind of environment where your leadership on utilization management, transition planning, and patient throughput will have real, visible impact from day one.
- You'll own the full scope of case management strategy: driving smart utilization management that protects medical necessity determinations and prevents denials; guiding transitions that shorten unnecessary length of stay, reduce readmissions, and keep patients satisfied; coordinating care so patients move through the system at the right pace and the right level; keeping the department squarely aligned with state, federal, and Joint Commission accreditation standards; and mentoring physicians, patients, and families along the way.

Required Experience
• At least 3 years of acute care hospital case management or healthcare leadership experience
• Active, unrestricted Registered Nurse (RN) or LCSW/LMSW license, currently licensed, certified, or registered to practice as required by law or regulation in the state of practice
Required Qualifications
Required:
• Bachelor's degree in Business, Nursing, or Health Care Administration (for RN candidates) or a Master's degree in Social Work (for MSW candidates)
• Active RN or LCSW/LMSW license for the state(s) covered
Preferred:
• 5 years of acute hospital case management leadership experience, ideally across multiple sites
• MSN, MBA, MSW, or MHA
• Accredited Case Manager (ACM) certification
Key Responsibilities
• Set the tone for department-wide operations, keeping patient throughput moving and reimbursement accurate for every service line
• Build and steer the hospital's Utilization Management Plan, letting data guide continuous performance gains
• Keep medical necessity reviews and revenue cycle work sharp, accurate, and aligned with CMS and organizational policy
• Champion timely, well-coordinated transition planning that keeps patients moving efficiently through their care journey
• Partner with physicians through ongoing education and feedback on utilization trends
• Take on additional projects and duties as the role evolves
